2026 Paperwork

Happy New Year!  Current clients please complete updated paperwork for 2026.   Thank you so much for trusting Therapy Steps with your child's therapy.  We are so excited for 2026 and all the progress that is ahead!  

 

Please note that new paperwork must be completed even if nothing has changed.   We ask for you to complete updated paperwork prior to your first appointment of the year to allow us time to complete any insurance authorization needed.  We greatly appreciate your prompt attention to this matter and please feel free to email therapystepsbilling@gmail.com with any questions.   If you have any issues  with online completion, you can also download/print the forms.
